SBAR
“Hand-off” communication will take place whenever there is a change in the patient’s caregivers. Caregivers include all clinical staff and medical providers. The SBAR format will be used. Hand-off communication will include at a minimum the following:
- S – the current Situation or problem
- B – a little about the patient’s Background
- A – your Assessment of the patient
- R – your Recommendation of what is needed for the patient
